Context

Implement this MVP work package under M7 - File Lifecycle & Health, following the managed-storage decision in #1.

Acceptance criteria

Provide manual and scheduled rescan of managed storage, distinguishing missing, changed and inaccessible files.
Reconcile physical folders including uncataloged entries; do not automatically ingest new files or implement watched-folder ingestion.
Serialize with imports/moves; stream hashing and expose progress/failures without changing added_at.
Test external moves, deletes, overwrites, permission failures and interruptions; permission failure is not proof of deletion.
