Context

Implement this MVP work package under M7 - File Lifecycle & Health, following the managed-storage decision in #1.

Acceptance criteria

  • Flag content-hash changes as Updated; timestamp-only changes do not imply new content.
  • Keep observed and acknowledged hashes distinguishable until user confirms re-baselining.
  • Reverify at acknowledgement to handle intervening changes; preserve added_at and document inability to recover old content.
  • Test repeated rescans, acknowledgement and invalid externally overwritten files; do not create versions.

Blocked by: #3, #46, #43
